Therapy Pricing and Billing Information

Please Note:

Therapy with Mariena Joy LCSW of Brainbow Forge, LLC is out-of-network for most insurance contracts as of 2022. We are in network for OHP and Western Oregon Advanced Health CCO.

As of 6/1/2022, Brainbow Fire will only provide superbills for new clients to submit for reimbursement for out-of-network plans. Due to time constraints, we will no longer bill out of network insurance directly.

Brainbow Forge, LLC recommends you check with your insurance company to ask about out-of-network benefits and how to file for reimbursement.

We offer multiple cost saving options such as: Billing health savings accounts, payment plans, reduced rates for private pay, and reduced rates for extended appointments.

 Mental Health Assessment (90-120 minutes)

All new clients using insurance or filing for reimbursement from insurance must complete a mental health assessment.

Clients paying out-of-pocket and NOT filing for reimbursement are not required to complete this assessment. They may choose to do so for diagnostic purposes.

 Rates for cash/private-pay

 

Clients with high-deductible insurance plans may want to consider paying the cash/private-pay rate below and submitting to their insurance company for reimbursement. Many insurance companies will reimburse up to 80%. Please check with your insurance company first to insure coverage.

Clients paying out-of-pocket and NOT filing for reimbursement are not required to complete the mental health assessment.

Why private pay? - Many clients will use private pay with no reimbursement through insurance to reduce hassle, skip having a diagnosis in their medical record, schedule more frequent/longer appointments than insurance would reimburse, or to reduce cost.

 

Rates for crisis, assessment, consultation, and late fees same as above.

 

Individual or Family Therapy (45-55 minutes)

Intensive Individual Therapy - EMDR only (150 minutes)

In the course of EMDR, some clients may wish to engage in intensive services to more quickly move through treatment. Intensive sessions available ONLY at cash rate.

Submitting for out-of-network reimbursement

 

Please be aware that you are ultimately responsible for payment of services. Clients with out of network insurance will be required to pay Brainbow Fire for the full amount of the service at the agreed upon intervals. Superbills for you to apply for reimbursement with your insurance company.

We strongly recommend you talk to your insurance company directly so you know exactly what your financial responsibility will be.

Here are some questions you can ask:

  • Do I have an out-of-network benefit?

  • Do I have an out-of-network deductible and if so, how much is it?

  • How much of my out-of-network deductible has already been met?

  • Do I have an out-of-network co-insurance payment for outpatient mental health services and how much is it?

  • Are there any services or diagnosis that are excluded from reimbursement? ***

  • Do I need a prior authorization for this care?

  • Are the benefits you've quoted me the same for in-person services and telehealth and if not, what are the differences?

  • What are the steps for filing for reimbursement?

 

Should payment of service be denied by the insurance carrier, you will still be responsible for payment in full to Brainbow Fire. You may contact Brainbow Fire with detailed information about why the claim was denied, and we will provide you with an amended superbill, if possible, to resubmit your rejected claim. Brainbow Fire will not do this more than once per claim. To avoid potential rejected claim, please contact your insurance company FIRST with the questions above and provide this*** information to Brainbow Fire.

 

Note for Current Out of Network Clients: Billing for out of network will continue through 12/31/2022. As of 2023, We will no longer bill insurance for you. Should payment of services be denied by the insurance carrier, the balance due will be billed to the client or representative as named in the billing agreement. Payment arrangements can be made. In some situations, insurance companies send payment for services to the client or insured. Any payment for services provided will be remitted to Mariena Joy LCSW of Brainbow Forge, LLC.
